you need three nightmares (any combination of wormfang and faceless) and a cleric or the denizen... It is up to you which creature your first nightmare exiles. The second nightmare needs to exile the first one, and the third nightmare needs to exile the second one, so that the first nightmare comes back exiling the third so that the second comes back exiling the first one so that the third one comes back exiling the second one......The infinite rotation will always trigger the clerics or the denizen (if at least one nightmare is a wormfang) so that you get infinite life or the opponent mills his library.
