puzzel box would be nice with underworld dreams
The Megrim/Underworld Dreams combo is deadly. Some constructive criticism would be either take out Kagemaro, First to Suffer or Demon's Jester or both. They do not go well together at all and in my opinion neither is necessary in this deck. Replace them with either Cephalid Looter, Cephalid Broker, Lumengrid Augur, or Words of Waste or up the number of Lore Brokers and Underworld Dreams in your deck. I thinks maybe a Seizan, The Perverter is in order. With his good friend Teferi's Puzzle Box. 2 More Underworld Dreams couldn't hurt, and I'd suggest you lose the Delirium Skeins and other self discard, focusing on things like Mind Rot, Mind Shatter, etc. Please check out my decks and comment.
