Lightning Helix and Lightning bolt would be included in the above requirements.

I recommend running four Forbidden Orchard, just in case your opponent doesn't play many creatures.

Would cards like Pestilence be considered direct damage, for the purposes of this competition?

No. For this I would consider only damage spells that target something, be it player or creature, as direct damage.

SO would that include cards such as pyroclasm? or volcanic fallout?

I did this for the random card deck challenge but I did use Repercussion as well and I think it qualifies for this. Here ya go.

I rather like this one, forced me to find a few new cards to use :)

Pyrohemia and Pyroclasm dish out the damage to creatures, Repercussion duplicates that damage to the creatures' controller. Meanwhile Fog Bank and Dawn Elemental, or any creature I control while Mark of Asylum is in play or Glory is in my graveyard, in conjunction with Pariah's Shield will prevent any damage dealt to me.

Cards like Haunted Dragon, Haunted Phantasm, and Varchild's War-Riders ensure that my opponent's continuously have creatures in play.

Nope, as they do not specifically target a player or creature. They hit everything.

Most of the creature abilities target something, be it a creature or player. Incendiary does as well.

Vigor + Arena. There are a few burn spells, but they could easily be changed out for more mana accelleration.
