So I see Quest for the Nihil Stone out there without any counters. I play Wrench Mind and my opponent has two cards in his hand and neither is an artifact. He discards two cards.

Do I have one counter on Quest for the Nihil Stone or two, i.e. does Quest for the Nihil Stone activate during every discard event --- for lack of a better phrase --- or on each card discarded? I think it's the former, but I'd like to know for sure, one way or the other. As you might imagine, this might make the difference between winning and losing. It'll also bother the usefulness of Liliana's Caress, Megrim, Wistful Thinking and Urza's Guilt.
I'd be much obliged to be pointed in the direction of a general rule for all these cards.

I believe you will get 2 counters. because as the card states " whenever an opponent discards a card..." which in effect also triggers when the opponent has 8 cards in play at the discard phase.

The card (and oracle) states "a card" so that would be per card.
