I'm going to split my suggestions into two sections. The first section will be for general deck enhancement, the second section will be specifically to modify in some timmy aspects. =)

Section I:

There is a new card called vampiric rites that is an enchantment that may fit better in this deck than Greed, and I would suggest looking at it. It might also combo nicely with Phyrexian Reclamation.

Bloodline Keeper is great, I'd almost suggest putting in a third. Actually, I would suggest it.

To combo with your exsanguinate, Guul Draz Assassin, Profane Command, and my suggested Drana (see section II), I would recommend maybe one or two Crypt Ghasts.

Alongside Crypt Ghast, I suggest putting in an Urborg, Tomb of Yawgmoth.

Urge to Feed! I love that thing, it looks terribly amazing. Keep it in and make sure she knows all of the fancy little tech things it can do!

I personally think I like Bloodflow Connoisseur better than Bloodthrone Vampire, it fits with your +1/+1 counter theme you're working with, and that thing can actually get freaking big really freaking fast, which means maybe I should put this comment in the timmy section....

On that note, depending if you're okay spending a little bit of money, I think Bloodghast would really help this deck too. What's not to like about a vampire that brings himself back in a deck that has a knack for sacrificing creatures?

Section II:

That Drana in the sideboard seems very much like a happy timmy card, and techie, and all around something that I find is a happy combination of fun and competitive.

Bloodlord of Vaasgoth oh my gosh I hate playing against that thing. How could a timmy not love playing that guy as a 6/6 that's also giving anything else you play later three +1/+1 counters?

Vampire Nocturnus is so freaking powerful, he makes everything big, your deck is only black, you're playing fewer lands than typical. My only stipulation is that you may want 3-4 if you play him, because a) he stacks, and b) people will want him dead.

My last suggestion for a nice timmy you could maybe put one or two of in here is Necropolis Regent. It seems to fit with what you've got so far.

Probably wouldn't need to. In test draws I consistently got about two to three lands.

My only thought is that you might want to see what works better between Dark Ritual, which is super flavorful, over something that would draw you cards, like Sign in Blood or Altar's Reap (Altar's Reap also very flavorful =) )

Centaur Battlemaster he's a five cost, but his heroic gives him 3 +1/+1 counters
There are a couple others that cost the same amount of mana and put 2 counters, but that's about it.

I've looked at Triton Tactics quite extensively before I saw it your deck, I know it's good, I basically looked at and didn't think it was helping do what you were wanting quite as much as some of the other cards. Definitely a good card, I won't deny.

I do think you might want a few more creatures, especially since your main goal of all your other cards is to pump creatures. If you don't have more out, killing them one by one is going to be super easy.

I'm not sure which spells to take out though. Probably the really high cost ones, starting with Incremental Growth and then to Increasing Savagery simply because of your mana base. Drawing sample hands, I have issues getting enough mana, so, I think maybe

-1 Incremental Growth
+1 Hero of Leina Tower
-1 Triton Tactics
+1 Battlefield Thaumaturge (or another small creature)

Otherwise, it's getting pretty good. =)

+2 Krark's Thumb (Would work really well with Stitch in Time)

Nothing in your sideboard currently is worth putting in.

Looking at this, the way you win is by spamming multiple turns, and punching with creatures. The creatures you punch with need to be big, not small, because in those extra turns you need to win. I would suggest, instead of small good punchers, you put small good defenders and huge punchers. Otherwise you'll be swept away completely. Path to Exile, though nice, is not going to do as much for you in this deck. It's going to give them land while you stall for time, which, in a sense, is giving THEM extra turns. Once you get turn spammy, then you shouldn't need to destroy their stuff, you'll be freaking punching with Aurelia. I'd suggest also putting in Gisella.

I'd also tell you to take out the Boros Mastiff, but I know how much you love the card. It's just that it's another small guy that's not going to help your deck run.

Also, Chromatic Lantern is absolutely needed if you plan on making Lighthouse Chronologist viable. He only levels up when you pay blue mana, so all your other mana won't help at all. Plus, you've got a ton of multicolored, costing two of something specific, which is going to hurt your speed immensely.

So, I'm assuming your goal is +1/+1 counters for massive attacks, and then making extra turns with Sage of Hours
The massive attack theme is solid. However, the extra turn strategy is not.
I think you could abuse Polymorphous Rush to no end.

Example

A creature on the battlefield with 5 counters.
Five mana (two blue)
Cast Sage of Hours
Polymorphous Rush
Hey, look, respond or I take another turn after this one. =)

Forced Adaptation is in the sideboard...why? It would be really useful in here. Triggers heroic, plus counters, plus bioshift. Sure, people can kill the thing it's attached to, but I'd be gunning for some of the stuff you'd really want to attach it to anyway.

I really don't like the white in here. The most useful thing it does is revive with Tethmos High Priest. Ajani, while cool, just doesn't seem to fit as much.

I'd suggest

Take out white
+1 Give / / Take
+1 Forced Adaptation
+3 Polymorphous Rush
- 1 Solidarity of Heroes
-1 Triton Tactics

Adjust numbers as needed once finished
