I would add temple bells or howling mines since they work well with underworld dreams.. If your facing control decks, you can make them discard by having over 7 cards in hand even without dreams and take damage from the megrims/caresses.... basically it punishes their hand size and rewards yours since you get your removal more readily.

I think that that the puzzle boxes are enough for the underworld dreams, and they also screw up controll decks. So I think i'll keep it like it is.
Maybe you should try Psychic Miasma to damage them more after they have no hand.

I don't know, I think that the quandary's a little pricy manawise for this deck.
