It seems there is just too much going on here. There is no theme or path for your deck to follow. With 5 colors, it may behoove you to go with a theme to reign in your ideas and focus your card pool.

First thing I noticed, you are running green but have absolutely no mana ramp(sans boundless realms, but that has cmc 0f 7), you have 36 lands and 2 mana rocks which will put you behind everyone else in mana. At the bare minimum, if you're going to run green, run something that will fetch you lands: Cultivate, Rampant Growth, Farseek, etc. And if you run more than two colors, Chromatic Latern is your best friend. Else, you will struggle to get the mana you need when you need it.

Second, from the creatures you are running, it almost feels like you want to run a zoo deck or a Naya type beasts deck, but I also see an idea in there for flickering creatures(conjerur's closet) and a splash of graveyard play as well. As I stated earlier, it may be easier on you to pick one, maybe two these themes and build around that, it will narrow your card selection and give you a more developed idea of what you want and where you want it in the deck.

Third, It's hard to recommend cards to use and/or remove without really knowing exactly what's going on. That said, some of my favorites that always seem to prove effective are as follows:

Urban Evolution: 3UG- Draw three cards and play and extra land.
-execellent for digging for answers and filling up your hand.

Animate Dead: 1B- Enchant creature in a graveyard and put it on your battlefield with -1/0.
-perfect graveyard plundering at a great cost. Great early game for people who are discarding and drop a fattie in their graveyard, great late game when you need answer and don't have fatties in your hand.

Sphinx's Revelation: XWBB- (instant) Draw X cards and gain X life.
-if you're running these colors and not running this, your deck is wrong.

Solemn Simulacrum: 4- When he ETBs get a land in tapped. When he dies draw a card.
-two card for one exchange and a nice chump blocker.

Sun Titan: 4WW- When ETBs and attacks, return permanent from graveyard with CMC 3 or less. Vigilance.
-This guy is a powerhouse, he attacks, he blocks and he get's you stuff back from the graveyard...even lands.

Lightning Greaves 2 cmc, 0 eq / Swiftfoot Boots 2 cmc, 1 eq: both give your guy haste, LG gives shroud, SFB gives hexproof.

Merciless Eviction: 4BW- Exile all of one category: Artifacts, enchantments, Creatures or Planeswalkers. Nuff Said.

Venser, the Sojourner: 3UW- He blinks, he makes unblockable, and his grand finale is exiling something with every one of your spell casts.

Garruk Wildspeaker: 2GG- Boosts mana, gives 3/3 beasts, Overrun effect.

Tamiyo, the Moon Sage: 3UU- Taps things for a turn, draws you cards, no max hand size and your spells bounce back to your hand. excellent control.

Domni Rade: 1RG- can get you beasts in your hand, makes creatures fight, a loooong list of awesome on an emblem.

Any of the Zenith cards: good effects and it returns to your library.

Cyclonic Rift: 1U- Bounce a nonland perm. 6U- Bounce all the nonland perms (except yours). This is just a game winner here.

Swords to Plowshares, Path to Exile, Oblivion Ring, etc. - Plain and simple and very cheap exile.

Hull Breach, Ancient Grudge, Shattering Spree, Disenchant, etc. - Getting rid of artifacts and enchantments on the cheap.

Return to Dust, Revoke existence, etc- When destroy is not an option or impossible. With the new Theros gods out, you will need the ability to either exile them as a creature and/or as an enchantment.

Mortify, Putrify- Great optional creature/artifact/enchantment killing.

Any of the Return to Ravnica charms: the better ones- Boros, Selesnya, Simic, and Rakdos. Really good option cards with good effects.

Restoration Angel: 3W- Flash, Flying, blink an non Angel creature on your board. Excellent for surprise blocks and amplified effects(look at you Thragtusk).

Tutors: Based upon the needs of your deck, these are very handy in filtering through your deck. (Demonic Tutor is the best)

Sensei's Diving Top 1 CMC, 1 to peek, T to draw : Excellent filiter card that gives you a peek a boo effect and draws you cards when you need it.

Jace Beleren: 1UU- draws you cards. Nuff Said.

Ral Zarek: 2UR- Tap/Untap effect, lightning bolts something, spin the wheel for a possible 5 extra turns. Great abilities.

Phyrexian Metamorph, Phantasmal Image: Cheap clone cards, always good.

Praetor's Counsel: 5GGG- Return all cards from your graveyard to your hand, no max hand size.
-Expensive, yes. But epically powerful. Pulling this off late game will make your opponents curse everytime.

Any of the new God Artifact enchantments: Some better than others but all real good.

Command Tower: Get one of any color in your Commander's color identity.

Reliquary Tower: No max hand size.

Kessig Wolf Run: XRG: +X/0 and trample.

Halimar Depths: top three card peek.

Temple of the False God: Tap for 2 mana if you have four or more lands.
-works great with Garruk Wildspeark :)

Tezzeret's Gambit: 3 U/P U/P: Draw two cards then proliferate.
-Not meant for every deck but this can be an explosive card if you play several planeswalkers or permanents that utilize +1/+1 counters.

Primal Bounty: 5G- Creature cast: get a 3/3 beast, Noncreature cast: 3 +1/+1 counters on creature, landfall: Gain 3 life.
-This will turn a game around almost instantly, this card will win you games, especially in one on one, this card is insane.

That was quite a list and not all inclusive. I tried to give a list of cards that will be good in almost any deck you play with the respective colors of the cards.

Fourth, research, experiment, trial and error, playtest and have fun. There are several sites out there that can be helpful in researching decks, cards and the like. One of the most powerful is Wizard's own Gatherer, not only do you have the full library of cards but you get oracle text, you get to see ever version of the card and reading the discussion section can actually be beneficial when it comes to hunting for ideas and synergy between cards. Cockatrice, tapped out are good sites for playtesting and fish bowling. Don't be afraid to switch cards back and forth and mix things up. I always like to throw in a WTF card into my decks for fun so as shake up games and cause some chaos. Have fun with it! Also, check out other people's decks, see what ideas other people came up with and see what you can do with the ideas.

Here is my list of decks: http://www.mtgvault.com/andywontdie/

In particular, here is my 5 color undying, persist deck: http://www.mtgvault.com/andywontdie/decks/undying-nightmare/

Hope that helped! Ok, I'm off my soapbox now :P

Take another look at your deck, you have several errors. Enormous Baloth is not a 3 drop. Leatherback is. Plus Glissa is too mana intensive for this deck. If you want to get the sac engine going you are going to need to put 4 of for birthing pod and 4 or for your mimic vat and adjust accordingly. Plus, having so few removal spells can be problematic. Massacre Wurn is a great card but again, it's mana intensive, a BBB casting cost will be hard to deal with if you get this stuck in your hand and can't cast it. Myr Superion is a amazing card for this type of theme, you should probably run more, and one more thought about Vengevine to remember is that yes, it is an awesome card to sac and dredge back but it will NOT triggger on brithing pod's ability as you are putting the creature, NOT casting as the text states. I like the idea, with a little work it could be a smooth running engine. :)

Plus, with Elemental Appeal + Koth of the Hammer, you'd be able to pay the kicker on Elemental Appeal, which is always fun. :)

Drop the Kuldotha Phoenix, you can't recurr him and he cost 5 to cast. elemental appeal is a better fit for that type of slot. You might want shatters for a quicker response to ratchet bomb. If you destroy while it has 0 counters on it then its useless to opponent, if you have to wait til next turn or more, he can sack in response to you attempting to destroying it. Plus shatter is instant speed vs manic vandal. Just some ideas.

Nice deck idea, my only advice would be to change out something for sign in blood, say like treasure hunt or see beyond just because they don't have the 2 life draw back or even forsee if you like that. Thanks for sharing.
