I had caress in there and took it out and there's not really anything i want to get rid of at this point to put it back in. As for Painful Quandary...i find haunting echoes to be much more effective for 3BB.
mire's toll, duress, bloodchiefs ascension all cards i would consider. If you want to get rid of huanting echoes and still exile pic up leyline of the void. I would put the sign in blood in your main board though, once people figure out what your running they will try to start top decking as fast as possible so itll help fill there hand back up and help you search for cards if you need them. Have you considered the mimic vat + liliana'a specter win condition? no cards in their hand you tap mimic vat when they draw and force them to discard. its extremely evil but very hard to pull off
What purpose does Haunting Echoes serve in this deck? It's certainly helpful I suppose, but it seems like a High-Cost card that doesn't actually influence their hand. Perhaps it would be best to toss in Psychic Miasma instead? That card costs 2 Mana I believe, and causes the player to discard a card. If they discard a land, Psychic Miasma returns to your hand. In other words, it'll force them to discard a non-land item, probably something useful. So, they'll either have to leave their hand filled with mana, or allow you to use it again. In my opinion, swapping Haunting Echoes for PM is a better choice. And, perhaps, it might be useful to add another 2 PM to your side deck by removing 2 Quag Sickness.
How about shimian specter, that can be a BRUTAL card if it is allowed to stick around for a while. If you ask me it is the best Specter there is.
I don't think it's standard though. Otherwise, it would be great.
