Ive found that fathom mage is fairly useless as you get basically a 1/1 for 4 mana (yeah the ability is good but hey thats what bred for the hunt is for am i right?)

may try to work in a few http://www.mtgvault.com/card/cloudfin-raptor/GTC/
they can be nice for getting damage through early game probably a good replacement for hydroform
plus with evolve it is super important to have a creature out asap so that its evolve ability can get triggered

also you need Vorel ..... duh... probably 2 of him

also a few more beefy creatures to help evolve your creatures during the late game.... maybe adaptive snapjaw or leyline phantom
along the same lines... maybe one less renegade krasis. yes it is good but realistically the only thing evolving him is going to be master boimancer and giant adephage (unless you add in snap jaw and leyline phantom). You need to make sure you are getting full use of his ability

i also agree with nimbus swimmer

also 18 mana for a 60 card deck (even with the key runes) seems a tad risky. i normally run a minimum of 20

Legions initiative can only blink your creatures once and then it remains exiled. There are other cards though that can blink your creatures. Otherworldly journey, liberate and flickerform to name a few, though I know there are others

first instead of the ceratok try http://www.mtgvault.com/card/bramblewood-paragon/MOR/

basically the same thing at a cost of only 2 (and it helps your mana curve

while leyline phanton is good for evolving all your creatures it is pretty expensive to cast. personally i would ditch it completely but you might want to keep like 1

the Dr has a point too. you want as many small creatures as possible so 4 cloudfin raptors and probably 3-4 simic initiates and 3-4 experiment one this will be out first and then grow big later hopefully

the simic and zameck guildmages are both ok but you could probably find better uses for those slots so probably nix them

souls might is also pretty expensive to cast... maybe lower the number of those? and add in other things that are low cost and give one +1/+1 counter

dont have time now but ill give more input later

a few righteous charge can be super helpful to finish off your oponent especially since you are normally attacking with at least 3 creatures.

I normally save it until turn like 5 or 6 where you should have at least 5 creatures. this means you are adding an additional 10+ damage. i have managed to swing 18+ damage out of nowhere with righteous charge

in general dont have "target creater gets +x/+x" cards, instead have "give target creature x +1/+1 counters

goblin war strike to replace lava spike

something to bring you up to 60?

raging goblins maybe
or rise of the hobgoblins to give you an outlet for mana later in the game

http://www.mtgvault.com/card/chancellor-of-the-forge/NPH/ is also a good mana dump if you happen to make it to late game while also giving you something if you start out with him in your hand

chain lightning could be dangerous if you play it while you have krenko and no chieftain out then a red deck would be able to kill him. unlikely but seems like it would make more sense to just put in a lightning bolt (added bonus of it being an instant)

you need to focus this deck down. you should really have 3-4 of most of your creatures unless they are very high cost creatures. none of yours are.
if you want specific cards to cut or add or replace with better versions/ comment back and i can help.

right off the bat though you want 4 righteous charge

a few goblin war strikes would be super helpful when you have krenko out and dont want to sacrifice anything. maybe also 1 or 2 less of your high cost creatures ( the dragon and vampire) in exchange for a few raging goblins to get things started and as fuel for your krenko. that or a rise of the hobgoblins to spit out a bunch of goblins at once (plus the ability to give your creatures first strike).
