Yes, tokens do in fact go to the graveyard. However, once they get there they cease to exist.

Keep a theme of "Whenever this deals damage put a +1/+1 counter on it". Then throw in 4 Curse of Stalked Prey to speed things up. Also throw in sources of making your vamps unblockable (Walks, intimidate, ect)
Instead of Into the Void, yeah I'd say so
Definitely replace unsummon with silent departure. It is the same thing, with the same mana cost, but it also has flashback... why not!

Well for one, godless shrine's are very expensive. Also for the fact that with them you will either take two life or have it come it tapped. You would do better with dual lands then with that for this deck. They are only good in beginning of game otherwise they are hindering. However, rogues passage can make one of you creatures unblockable for a turn, very important since your one creature with exalted will be very strong and then unblockable to top it off for mass damage.
You should take out the ajani and put in two Vault of the Archangel instead. I'd also say take out two Godless shrine's for two Rogues Passage's.

How so? I love the spy for mill decks, I use him in mine, but I can't think of a reason why he fits in this one.
I love Wild Defiance. I recently made a vintage/legacy unblockable deck featuring Invisible Stalker, Incursion Specialist, and Blighted Agent. I run Wild defiance and 1 drop boosts like giant growth and ground swell then double strike with assault strike or double cleave. It's a quick winner for a consistent turn 3-4 win. With wild defiance out I can play assault strike and ground swell for an unbloackable +10/+10 and double strike, ahah.
Love it. And yes, since Wild Defiance gives +3/+3 each time a creature is targeted by a(n) instant or sorcery spell, using Martial Glory would in fact give it +9/+9. Since you would be targeting twice; whether you target two creatures or target one creature two times.

I've actually had a Sacrifice deck just like this one. Different of course, but same idea of using sacrifice benefits and gain creature control. I like your take. Since then I've gone through many different versions of The Sacrifice deck and I love them all.

I know about both those cards and I would have to disagree. I'm looking for quick aggro gain, soon as the 1/1 human tokens touch the battlefield (including the temporary thatcher ones) I will sacrifice them for the gain of the vamps. Deal damage, then fling it for the final blow.
